Description: To provide cohesive, consistent, and comprehensive care to the Pediatric Diabetic population through the coordination and management of multidisciplinary teams at multiple clinical sites. To improve outcomes for the Pediatric Diabetic population by providing Case Management and diabetic educational services.

Essential Functions:
  • Design, implement, and maintain the clinical and educational programs/activities for Peds diabetic patients in the NW region.
  • Coordinate and manage the multidisciplinary diabetes team for consistent and comprehensive standards of care for Peds diabetic patients. Coordinates the actions of the multidisciplinary team with a diverse set of goals and clinical backgrounds.
  • Develop, implement, monitor and analyze patient outcome and quality assurance measures to assure effectiveness of overall program. Assures that the program meets community and national standards (i.e., HEIDS guidelines).
  • Provide direct care to members and families including Case Management of chronically ill patients with varying social, economic, emotional, developmental and medical issues which may affect compliance with the plan of care.
  • Assess, plan, implement, and evaluate effectiveness of individual patient care plans based on the nursing process. Develop and apply nursing protocols as deemed appropriate by the diabetes team. Provides assessments, telephone advice, medication adjustments and patient instruction using protocols and independent nursing knowledge unique to the CDE RN skill set (i.e., insulin teaching, insulin pump training appraised of clinical best practices based on research driven evidence.
  • Coordination of the administrative functions associated with providing multidisciplinary services to patients at five (5) clinical sites. Maintaining effective communication linkage with five (5) clinical sites and team members.

    Basic Qualifications:
  • Minimum three (3) years of current (in the past two (2) years) experience in diabetic care with an emphasis on Diabetic Case Management plus one (1) year -current outpatient experience.
  • Minimum one (1) year of classroom/small group, presentation, facilitation and process tracking which can be taken concurrently.
  • Minimum one (1) year of experience in Pediatrics with preferred experience with Type I diabetics.
  • High School Diploma or General Education Development (GED) required.
    License, Certification, Registration
  • Current Oregon and Washington RN license required. License in state where assigned required upon hire/transfer, application for license in secondary state required within two (2) weeks of hire/transfer and must be obtained within six (6) months.
  • Certified Diabetes Educator (CDE) within one (1) year of hire.
  • National Provider Identifier (NPI) required prior to employment start date.

    Additional Requirements:
  • Experience in diabetes teaching, nursing advice, pump management and insulin instruction.
  • Must be CDE certified and maintain certification throughout employment.
  • Must be a Registered Nurse with current Oregon and Washington license.
  • Current and thorough understanding of Diabetes treatment management and education including dietary modifications, exercise and medication regimens.
  • Knowledge of user of a variety of glucometers.
  • Working knowledge of Insulin Pumps and subcutaneous insulin injection techniques.
  • Knowledge of Pediatric Developmental stages and ability of children to learn self management of diabetes according to their developmental level.

    Preferred Qualifications:
  • Experience with multiple Kaiser Permanente care settings and computer systems.
  • Knowledge of EPIC system.
  • Prefer knowledge of KP computing systems including EPIC.
